Forms for Download

The IPA has determined that nearly all filings will be submitted online only. This decision was made partly in response to Covid and partly to gain all the efficiencies that an online system can provide to the business community. Therefore, there are only limited forms available for download here. The only paper filings that IPA will accept are: i) the initial Application for Incorporation of a Company; ii) the Application for Registration of a Business Name; iii) the Application for Incorporation of a Business Group; and iv) the Intent to Incorporate an Association. If you submit a paper form for one of these filing IPA will assist you in establishing an online account so that you can then manage your entity in the future.
