     4. Complete the forms 

     5. Sign the form

     6. Scan the completed form as 200dpi PDF document

     7. Send an email to EFF email address and attach the completed form

     8. For each printed page of form lodged a compulsory processing fee of K2 per each page will be deducted from the lodging client’s EFF account.

     9. For every megabyte of the attachment send by email, K0.45 will be deducted from the EFF account.

     10. Maximum attachment file size must be less than 3MB per email.
